Why I do not adjust extremities often

As a chiropractor, my primary goal is to provide effective and safe care to my patients. While spinal adjustments are a fundamental part of chiropractic care even if I don’t always adjust, you may wonder why I don’t adjust extremities like the shoulders, elbows, wrists, hips, knees, or ankles nearly as often. over the course of this article, I’d like to touch upon the importance of joint stability and why I prioritize patient safety by refraining from adjusting extremities.

Understanding Joint Stability:
Joint stability refers to the ability of a joint to maintain proper function and resist excessive or abnormal movement. We also should consider the difference between joint and tissue flexibility vs mobility. They’re the same thing? I’d beg to differ. Think of flexibility as passive range of motion (the capacity for the joint and/or tissue to move not under your control) and mobility and active range of motion (capacity in the joint and/or tissue that you can actively control). If a joint or tissue has significant flexibility without control, there isn’t much stability in that joint, which can easily lead to injury. On the contrary if joint or tissue has more mobility and control through its full range of motion it theoretically should have more stability. In some cases, extremities can already be compromised due to injuries, ligament laxity, underlying conditions, poor movement patterns or movement organization. Adjusting these unstable and/or injured joints may exacerbate the problem and potentially lead to further instability, discomfort, or even further injury.

Preserving Natural Stability Mechanisms:
Extremity joints are naturally equipped with built-in stability mechanisms such as ligaments, tendons and muscles. These structures work together to maintain joint integrity and provide stability during movement. If these structures are stretched or worked passively regularly or not put under load and tension this can lead to a lack of stability. By avoiding adjustments in extremities, I respect and preserve these natural stability mechanisms, allowing the body to heal and adapt naturally.

Individual Variations and Complexity:
Each individual’s extremities have unique anatomical and biomechanical characteristics. Yes, we all basically have the same “stuff”, however how that stuff develops isn’t the same across the board. This complexity requires a thorough understanding and specialized training to safely and effectively perform adjustments. Without proper expertise, attempting adjustments on extremities can carry an increased risk of complications or unintended consequences.

Alternative Treatment Options:
While I may not adjust extremities often, it’s important to note that I can still provide effective care for these areas. My practice includes a range of therapeutic modalities, such as soft tissue techniques, therapeutic exercises, stretching and mobilization techniques which can address extremity complaints. These approaches can help improve joint function, reduce pain and promote healing without the potential risks associated with extremity adjustments.

Collaborative Approach:
When patients present with extremity complaints or injuries, I take a collaborative approach by working closely with other healthcare professionals, such as orthopedic specialists, physical therapists or other sports rehab doctors. This interdisciplinary approach ensures comprehensive care, appropriate referrals and a tailored treatment plan that addresses the specific needs of the patient’s extremity condition.

Prioritizing patient safety and promoting joint stability are the main reasons why I choose not to perform adjustments on extremities. By respecting the body’s natural stability mechanisms and utilizing alternative treatment options and working to establish strength and control, I can provide comprehensive care while minimizing potential risks.
